There are 2 types of unincorporated businesses:
- Trade names (or Sole Proprietorship) - Operated by a single declarant.
- Partnership - Operated by two or more declarants.
A declarant can be an individual, legal entity or other entity.
What you need to request, amend or dissolve a trade name or partnership
- Your AMA membership card
- One piece of acceptable personal identification
- Completed trade name or partnership form (select your form below; can be completed at time of filing)
