What to do to order a death certificate
- Check your eligibility to apply.
- Visit one of our 18 retail centres conveniently located around Alberta.
When you come in, remember to bring:
- Your AMA membership card
- One piece of personal identification
- Your completed Ordering Death Vital Statistics form
Please note:
- The death certificate/document should arrive by mail 7-10 days after you place your order. If you require it sooner, you can pick it up in person. Ask an agent for details.
- When the event did not occur in Alberta, you need to contact the jurisdiction where the death took place.
- You can also mail in your request. Complete the Vital Statistics application form.
There are 3 types of death certificates:
Death certificate - Framing size
- 21.5 cm x 17.8 cm
Contains:
- Name
- Sex
- Age
- Marital status of deceased
- Date, time, and place of death
- Usual place of residence
- Registration number and registration date
Copy of medical
certificate of death
- 21.5 cm x 28 cm
Contains:
- Medical cause of death
- Name of attending physician or medical examiner that completed the medical certificate of death.
A photocopy of the medical certificate of death is completed at the time of death.
Copy of registration of death
Certified photocopies of original death registration, not the certificate itself
